* feat(gravatar): accept pre-computed MD5/SHA256 hash as input

Gravatar now passes hash inputs directly to the avatar URL instead of
re-hashing them, avoiding raw email exposure in the URL. Emails are
hashed with SHA256 (Gravatar supports both MD5 and SHA256). A new
is-hash utility detects 32 or 64 hex-char strings, and auto-routing
maps hash inputs to the gravatar provider.

Closes #430, related to #19
