v1.0.3: spanning progress indication (#27)

Multi-minute tsarina spanning runs now emit progress messages on stderr
instead of going silent during the scoring phase. Library stays quiet
by default (opt-in via on_progress callback); CLI wires the stderr
printer.

- spanning_pmhc_set: new on_progress kwarg, 3 callpoints
  (pre-scoring count, post-scoring elapsed, post-pivot summary).
- cli_spanning.handle: passes stderr-printing callback. Stdout stays
  reserved for the CSV payload.
- +5 tests (silent default, 3-message callback contract, accurate
  counts, handler wiring to stderr).

207 passed. Closes #24.
