Expose MutantTranscript on EXON_SKIPPING candidates (#305) (#348)

The splice-outcomes prototype's docstring has promised since day one
that it'd be "re-expressed as a thin layer over MutantTranscript
once #271 lands." #271 landed in 4.7.0. This is the thin-layer
plumbing for the EXON_SKIPPING case, which is the only outcome that
already builds a full MutantTranscript internally.

- Add mutant_transcript: Optional[MutantTranscript] field to
  SpliceCandidate (default None for back-compat).
- _build_exon_skipping_candidate now threads the internal
  _build_exon_skip_mutant_transcript result out to the candidate
  so consumers can reach cdna_sequence / edits / annotator_name
  without re-deriving from the coding_effect.
- NORMAL_SPLICING and the stubs (INTRON_RETENTION, CRYPTIC_*) stay
  at None. NORMAL_SPLICING's coding impact is already fully
  captured by .coding_effect; the stubs need genomic-FASTA
  ingestion (#296) to populate.

Tests cover the new field for EXON_SKIPPING (MutantTranscript has
a populated cdna_sequence, one deletion-shaped edit, right
annotator_name) and confirm stub candidates are still None.
