feat: add stream/2 to Notes, Tasks, Meetings, and Threads

All four modules now expose a lazy `stream/2` (with optional params)
backed by `Client.paginate/3`, consistent with Records and Entries.
Multi-page and mid-stream error tests added for each. The pagination
section in the top-level Attio moduledoc is updated with a reference
table covering all six streaming modules.
