Replace vendored COFFEE with git dev-dep on cgevans/coffee fork

Drop the tests/coffee_vendor/ tree in favor of a pinned git dev-dependency
on the cgevans/coffee fork, which gates the polars-backed file-input path
behind a feature so default-features=false keeps polars (and its transitive
cloud of arrow2/multiversion/comfy-table/etc) out of the dev build.

COFFEE pins ndarray 0.16 while equiconc is on 0.17, so the benches/tests/
examples carry both versions: a `ndarray_coffee` alias for the 0.16 types
COFFEE's API requires, alongside equiconc's 0.17 types. instrument_large
and instrument_xl no longer report a COFFEE iteration count because the
upstream Optimizer has no public accessor for it.

With the vendored Apache-2.0 code removed, revert the crate license field
from "BSD-3-Clause AND Apache-2.0" back to pure "BSD-3-Clause".
