Flip default annotator to protein_diff (#329)

After #318/#319/#320/#321 closed the last known correctness bugs,
``protein_diff`` is strictly at-or-better than ``fast`` on every
case the parity + divergence suites cover. Flip the registry
default so new callers pick up HGVS-canonical output (3'-rule for
ambiguous indels, FrameShiftTruncation specificity, correct stop-
loss and alternate-start classification) without opting in.

``fast`` stays available (``annotator="fast"`` or
``use_annotator("fast")``) for performance-sensitive pipelines
and byte-for-byte compatibility with 2.x output.

Update the two tests that pinned ``"fast"`` as the default.

Bump minor version to 4.1.0 since this changes default behaviour.
