fix: loosen PreCompact decision detection to reduce false-positive blocks (#59)

- remove `/\buse\b/i` and `/\bapproach\b/i` patterns from decision_patterns in plugin/scripts/lib.js (lines 68-81). these patterns match nearly every assistant turn ("use the Read tool", "this approach") and cause false-positive blocks.
- raise threshold from 1 to 3 for both auto and manual triggers in plugin/scripts/pre-compact.js (line 60). sessions with only 1-2 decision words no longer block on manual compact; 3+ true decision patterns required to block.
- update tests to reflect new pattern removals and threshold: remove "use"/"approach" from positive test cases in lib.test.js, adjust pre-compact heuristic tests to require 3 decision turns to block, add regression tests for benign "use"/"approach" language and 1-2 decision words on manual trigger.
