feat(server): support externalAuthProviders via environment variable (#8978)

Add 'externalAuthProviders' to the objectKeys set so the env config
loader JSON-parses the value instead of treating it as a raw string.

Usage:
  MEDPLUM_EXTERNAL_AUTH_PROVIDERS='[{"issuer":"...","userInfoUrl":"..."}]'
