fix: commandUsage syntax error and test mock for dbUtils migration

- Remove duplicate ); in commandUsage.js (syntax error from dbUtils refactor)
- Mock dbUtils.js in commandUsage test (module switched from pool.query to queryWithLogging)
- Make API server startup failure fatal (rethrow so Railway restarts the container)
- Add test coverage for API server startup failure in index.test.js
