fix(release): npm whoami and --workspaces=false

Log which npm user `NPM_TOKEN` maps to before upload. Pass
`--workspaces=false` so `npm publish` targets the package directory only
and does not walk the monorepo workspace graph (sibling resolution from
the registry). Expand the E404 comment to point at org/token access.

fix(cli): document Node 18+ in README

Align the intro with `package.json` `engines`.

Example:

```json
"engines": { "node": ">=18" }
```

docs(sidecar): clarify assert-published-cli-dep has no npm HTTP

The script only validates `package.json`; the name “published” means
“suitable for a tarball”, not “already on npm”.
