refactor: unify mock_claude command dispatch via handle_prompt

Consolidate /echo, /exit, /env, /sleep into a single handle_prompt
dispatcher with SessionContext + PromptResult so commands work
uniformly in both --print and interactive mode. The interactive loop
calls handle_prompt first, then falls through to interactive-only
commands (/busy, /permission, /hook, /run, /break-*).
