fix(rpc_notify_postgres): auto-reconnect with health check polling

Postgres Dart library does not fire onDone/onError on channel subscriptions
when the connection is terminated externally. Replaced event-driven detection
with periodic isOpen polling (default 10s, configurable). On connection loss,
triggers backoff reconnect loop and re-subscribes to all active topics.
