* feat(pair-ui): add visible back button in header (PT-1220 AC 6-8)

Add a \←\ back arrow button to the header bar, visible on pages 2-6
and hidden on page 1 (Welcome). This provides a discoverable back-
navigation affordance on desktop platforms (Windows, Linux) that lack
a hardware back button.

Changes:
- index.html: add \tn-back\ element in header with \.header-row\ wrapper
- styles.css: add \.header-row\ and \.btn-back\ styles (borderless icon)
- main.js: add DOM ref, visibility toggle in \_updateStepper()\, click
  listener calling \
avigator_.back()\
- requirements: add PT-1220 AC 6-8
- design: add visible back button subsection
- validation: add T-PT-1220d test case and traceability entry

* fix: use history.back() for back button, fix test prereqs

Address PR #758 review feedback:
- Use \history.back()\ instead of \
avigator_.back()\ to correctly
  pop the history stack and stay in sync with platform back action
- Update design doc to match implementation
- Add prerequisite step in T-PT-1220d for reaching page 4

* style: remove redundant .btn-back.hidden rule

The global .hidden utility class already applies display:none !important,
making the button-specific override unnecessary.
