* Add utf8_by_default terminal behaviour flag

* Use ambient UTF-8 for default terminal output

* Keep UTF-8 ambient across ASCII writes

* Cover 7-bit control framing with UTF-8 text

* Translate legacy glyphs in UTF-8 mode

* Cover legacy fallback and UK UTF-8 mapping
