docs: swap integration guide with UI patterns and best practices

New dart/doc/swap_integration.md walks through the
quote → approval → execute flow with an explicit focus on
what to render at each step. Covers:

- Field-level inventory of SwapQuote / ApprovalPreview /
  SwapResult and what each maps to in a UI
- Sheet mockups for the quote, tight-default approval,
  unlimited-approval warning, and success states
- Quote expiry + auto-refresh patterns
- Price-impact warning thresholds (1% yellow, 5% red+confirm)
- Why approval-to-exact is the safer default and how to
  frame the unlimited opt-in
- Error-code table with recommended UI responses per code
- Slippage tuning, cancelation, pre-ship test checklist

README gets a Swap section and an entry in the API table
pointing at the guide.
