feat(budgets): Per-request query budgets and documentation examples (#16)

* feat: per-request query budgets and documentation refresh

- Track maxQueries and maxTotalDurationMs per requestId with LRU cap and db.request.budget events.

- Wire budgets through SlowQueryDetector, LoggerSink, and DetectorEvent union.

- README: postgres.js + wrapQueryFn examples, runWithDbContext, sink notes, Prisma subpath.

- docs/index.html: correct @olegkoval/queryd/prisma import, requestBudget in samples, postgres.js panel.

- Third code panel for string-SQL wrapQueryFn + runWithDbContext.

- Flatten body/header (no radial gradients or backdrop blur), softer tokens and shadows, gentler reveal motion.
