* package.json
  * removed use of `node-helpers`
  * added use of `generic-request-options`
  * added use of `request-as-bluebird`
* added additional documentation and toc to readme.md
* changed signature of
  * callApi( req, user_options, callback ) => getApiCallback( user_options, request_headers, callback )
  * getApiPromise( req, user_options ) => getApiPromise( user_options, request_headers )
* added a test for getApiCallback
* added additional test fixtures
