doc: define npat: and npaa: prefixes for trust/account state IRIs

npat: = http://purl.org/nanopub/admin/truststate/
npaa: = http://purl.org/nanopub/admin/accountstate/

Updates the Identity Model section with the prefix declarations and
replaces all literal full-URL forms in turtle/sparql examples with the
prefixed shorthand. Java code samples reference NPAT.NAMESPACE /
NPAA.NAMESPACE constants (also a hint that the implementation should
add a vocabulary class).
