feat: migrate ouroboros-mcp plugin from marketplace (#28)

move the ouroboros-mcp plugin from dangernoodle-marketplace/plugins/
into this repo under plugin/. includes skills (persist, recall,
triage), agents (backlog-manager, knowledge-explorer), hook scripts
(kb persistence, context injection, staleness warnings), installer,
and full test suite (82 tests, node:test, zero npm deps).

- drop version from plugin.json so cache keys on source commit sha
- add plugin-tests job calling reusable plugin-test.yml workflow
- document plugin/ layout in CLAUDE.md
