#305: rewrite splice_outcomes exon-skip builder on MutantTranscript + shared classify_from_protein_diff. Removes _ExonSkipFrameshiftEffect shim and four ad-hoc helpers (-268 lines). Boundary-codon math, start-loss detection, and in-frame/out-of-frame distinction fall out of the protein diff naturally. Net -100 LOC. Introduces the shared classifier that stage 3d will use. 614 tests pass byte-for-byte.
