[WIP #271 stage 3d] SequenceDiffEffectAnnotator scaffolding + SV design notes (#310)

WIP #271 stage 3d scaffolding + user-facing effect-annotation guide. Ships the dormant SequenceDiffEffectAnnotator class (NotImplementedError stubs, not registered), the parity test harness (skipped), new docs/effect_annotation.md covering the four-primitives model and SV roadmap, MutantTranscript SV-extensibility docstring, pytest.mark.parity marker. No behavior change. 606 tests pass + 1 parity skip; Coveralls 503 on 3.11 is external.
