* docs(communications): add Creating your first thread guide

Add communications tutorial for threading via MedplumClient: thread header,
provider-patient messages, optional inResponseTo reply, and verification
using existing queryMessagesInThread examples. Document Medplum App as
inspector and optional manual create (with caveats), prerequisites including
register link and resolved Patient/Practitioner references, atomic transaction
Bundles for production, and link to FHIR batch requests. Examples live in
messaging-examples.ts; sidebar and communications index updated.

- Move console.log out of extracted blocks; use void after end-block markers
- Inline partOf/inResponseTo refs in optional reply snippet
- Subject only on thread header; duplicate category on child messages
- Doc: note category on messages and optional subject on children
