project: enforce sphinx warning failures in doc build scripts

Generated Sphinx doc and manpage invocations now fail on warnings so
local and CI docs builds catch regressions immediately. Post-processing
normalises existing scripts to add --fail-on-warning when missing, and
tests/changelog are updated to reflect the behaviour.
