fix ScanContract off-by-one, add tests, docs, CI

- Fix ScanContract missing a valid 11-byte dispatch pattern when the
  bytecode length equalled the pattern length (len > 11 -> >= 11).
- Add godoc for Abi, AbiIO, MethodPrefix, ScanContract, AbiList,
  LookupAbi and a package doc.
- Add scan_test.go covering multi-entry scan, the off-by-one regression,
  embedded-8063 selectors, empty/no-match input and unknown lookups.
- Rewrite README.md with Go and JS usage, limitations, and badges
  (GoDoc, CI, Coveralls, Go Report Card).
- Add GitHub Actions workflow: vet + race tests + Coveralls upload.
