Stage 2 of #271: apply_variant_to_transcript builds MutantTranscripts from Variant + Transcript, with reverse-strand and mitochondrial codon table handling. Returns None for intronic/non-coding/splice-spanning cases. 584 tests pass. Next: sequence_diff annotator wiring Variant.effects(annotator=...).
