EIP-6963 UUID: fresh per page load, not per install

The previous guidance (and dartdoc) told implementors to persist the
UUID across app launches. That's wrong — EIP-6963 scopes it to "the
lifetime of the page"; the field that identifies a wallet stably
across loads is rdns, not uuid.

Updated Web3Api.injectionScript dartdoc, doc/webview_integration.md
prerequisites + all code samples + the troubleshooting entry + the
end-to-end example. No code changes — the script works with any
valid UUIDv4, this is purely correcting how the host is told to
generate it.
