refactor: batch-only MCP surface for kb and backlog tools (#21)

- get/put/item/plan now accept ids[] or entries[] collections only
- single-item shape removed; callers wrap in one-element arrays
- shared kb.WriteBatch for cli stdin and mcp put paths
- trim serverInstructions and tool param descriptions
- add tools/list token footprint benchmark with tiktoken cl100k_base
- snapshot tools_list.json for visual diff in future prs
