ci: use github app token for release push past branch protection

- release.yml now mints a short-lived installation token via
  actions/create-github-app-token and uses it for checkout + git push,
  so the release commit/tag can land on master while master stays
  protected against direct human pushes (add the app to the bypass
  list and configure RELEASE_APP_ID + RELEASE_APP_PRIVATE_KEY secrets).
- ci.yml scoped to pushes on master and PRs targeting master, so the
  CI badge reflects master and feature-branch pushes don't burn CI.
- README: restore the npm monthly downloads badge.
