doc: update space repo plan with NPID/SPACEIDHASH identity model

Space identity is now defined as NPID/SPACEIDHASH, where NPID is the
origin nanopub's content-addressed artifact code. This eliminates
conflicts by construction and roots authority in the immutable origin
nanopub (which declares the initial admin set). SpaceRegistry maps,
persistence triples, and detection logic updated throughout.
