fix: narrow Response.t error type from any() to concrete union (#74)

Replace `{:error, any()}` with a named `error()` type that is the
union of `{pos_integer(), map()}` (HTTP error responses) and
`Exception.t()` (network/transport failures). Add moduledoc explaining
both error shapes.
