jae/backlog notes split (#16)

* feat: split backlog item description into terse + notes

- add notes column to items (migration v5)
- item tool accepts notes param, enforces 500-char description cap
- item get/list accept verbose flag controlling notes visibility
- response on create/update includes {id, action} pattern

mirrors the kb document split shipped in c4a22ab. backlog item
descriptions get loaded into agent context on every list query;
splitting narrative out of the always-visible field cuts cost.

* fix: log content-cap rejections for OU-24 monitoring

- strip temporary OU-24 debug logging
- log project/title/content len/notes len on cap rejection only
- add regression tests for cap behavior
