refactor: redesign file output with async pipeline and full test coverage

- introduce a shared async write pipeline for file outputs
- refactor file session around async append, rotation and retention
- align JSON output with logora-socketio instruction payloads
- replace formatter-based JSON flow with instruction factory and serializer
- refactor text output around a dedicated renderer
- reorganize internal core modules for clearer responsibilities
- add flush and close lifecycle support to file writers
- rewrite unit tests to cover the new async architecture
