fix(ci): use RELEASE_PLEASE_PAT to allow PR creation (#65)

GitHub Actions is not permitted to create or approve pull requests with
the default GITHUB_TOKEN. A personal access token stored as
RELEASE_PLEASE_PAT is required to bypass this restriction.
