Fix accuracy issues in skills architecture documentation (#4759)

* Fix accuracy issues in skills architecture documentation

Address review feedback from #4758:
- Correct SQLite schema to show actual table names (entries,
  installed_skills) with accurate columns and constraints
- Clarify that GIT_TOKEN is an unscoped fallback, not host-scoped
- Fix nonexistent path_resolver.go reference to types.go
- Add full name validation rules (start/end alphanumeric, no
  consecutive hyphens)
- Use distinct indigo color for Skills node in architecture map
  to avoid confusion with Core Understanding purple

- Use distinct emoji (🔷) for Indigo legend entry to avoid collision
  with existing Blue (🔵) entry
- Remove nonexistent --clients plural flag and --clients all mode from
  Multi-Client Support section (only --client singular exists)
- Add missing oci_tags table to Storage schema section
