Bump version to 2.2.1

Patch release:

* #215, #246: ref-allele mismatches now raise a dedicated
  varcode.ReferenceMismatchError with an actionable message that
  names the likely causes (wrong genome build, germline allele in
  ref, strand confusion) and points at the raise_on_error=False
  escape hatch. The exception subclasses ValueError so existing
  callers of the Failure-fallback path keep working.

No behaviour changes beyond the improved error.
