Prepare JSON conformance fixtures for external ports (#28)

* 📦 Ship conformance fixtures with @umpire/json package

Add `conformance/` to `files` so fixtures are included in the npm
tarball; external Kotlin/Python/Dart ports can now download them via
`npm pack @umpire/json` or reference the unpacked directory without
cloning the whole repo.

Add `conformance/index.json` as a flat manifest listing every fixture
and failure file with its id, path, and description so consumers can
discover the set programmatically rather than scanning the directory.

* 📝 Add port runner pseudocode to conformance README

Adds a "Writing A Port Runner" section with a language-neutral pseudocode
loop so Kotlin/Python/Dart implementations can consume the fixtures
without needing Node.js or any JS tooling.

Also documents the exact shape of expectedAvailability entries,
including when valid/error are present vs omitted.
