feat: split kb content into terse content + optional notes (#14)

- add notes column (migration v4); not indexed in fts5
- put accepts notes, enforces 500-char hard cap on content
- put returns {id, action} instead of {id, ok}
- get/search accept verbose flag (default false) controlling notes
- default limits lowered from 50 to 10
- server instructions trimmed + verbose discipline line added

reduces token cost on every subagent context injection while
preserving narrative rationale for explicit "why" queries.
