feat: Wave 2 benchmarks -- Ruby, Kotlin, PHP, Perl, Lua, C++ + scanner fixes

Add 6 language benchmarks with patches, configs, and baselines:
- Ruby: rack/rack (20 markers, 3 spec files)
- Kotlin: square/okio (20 markers, 2 test files)
- PHP: symfony/console (20 markers, 2 test classes)
- Perl: mojolicious/mojo (20 markers, 2 test files)
- Lua: lunarmodules/luacheck (20 markers, 2 spec files)
- C++: fmtlib/fmt (20 markers, 2 test files)

Scanner fixes discovered by benchmarks:
- Ruby: isRubyTestFile now matches spec_*.rb prefix pattern
- Ruby: extractRubyMarkersFromFile now consumes pending markers
  on "it" blocks (not just "def test_" methods)
- Lua: isLuaTestFile now matches *_spec.lua and spec/ directory
- C++: isCppTestFile now matches *-test.cc (hyphen) and test/ dir

Total scanner bugs found and fixed by benchmarks: 7
