Address Copilot PR review comments on Rust gatekeeper

- Split DefaultBodyLimit layers to match TS express middleware scope:
  json_limit (4 MB default) on every JSON route, upload_limit (10 MB) on
  /ipfs/text and /ipfs/data only, unbounded on /ipfs/stream. Previously
  the whole bounded router used max(json_limit, upload_limit) so JSON
  routes accepted 10 MB instead of 4 MB.
- Fix "did not found" -> "DID not found" across resolver.rs and store.rs
  so log output matches the TS wording.
- Drop dead "let _ = metric" lines in Metrics::new; those fields are
  already stored on Self.
- Rewrite AUDIT_REPORT.md executive summary as a before/after table so it
  reflects the current post-fix state instead of listing problems that
  have already been addressed.
