fix(docker): reduce macOS credential cleanup race and fail fast on missing creds

Bump the credential temp-file cleanup timer from 10s to 60s to cover
slower Docker Desktop / OrbStack cold-start paths where the container's
init-docker.sh cp could miss the file.

Add a fail-fast check on macOS when extract_macos_credentials returns
None and no .credentials.json exists on disk, replacing the confusing
"Not logged in" error that would otherwise surface inside claude later.
