Emit readable HTTP access lines in Rust gatekeeper

tower_http's TraceLayer logs via a span and its default on_response only
shows status+latency, so the container produced useless lines like
"finished processing request latency=0 ms status=200" with no method or
path. Replace it with an axum middleware that writes one info line per
request including method, path, query, status code, and elapsed ms.
