Fix immuclient scan to support listing all keys without prefix

Allow running 'scan' with no arguments to retrieve all keys from the
database. Previously, scan required exactly one prefix argument, making
it impossible to list all keys when the prefix was unknown.

Changes:
- Accept optional prefix argument (0 or 1 args instead of exactly 1)
- Handle empty/missing prefix by sending nil to server (matches all keys)
- Add tests for scan-all-keys behavior
