feat(react): export valueSetElementToCoding and codingToValueSetElement (#8933)

* feat(react): export valueSetElementToCoding and codingToValueSetElement

Exports `valueSetElementToCoding` and `codingToValueSetElement` from
`@medplum/react` as public API, enabling callers to safely convert
between `ValueSetExpansionContains` and FHIR `Coding` without carrying
over non-Coding fields such as `designation`.

Updates both `medplum-provider` and `medplum-health-gorilla-demo` to
import and use `valueSetElementToCoding` in their diagnoses `onChange`
handlers, replacing the inline projection from the hotfix PR.

The inline projection in PR 1 was a self-contained hotfix that avoids a
dependency on an unreleased package version. This PR standardises the
pattern once the package has been released.
