refactor: extract internal/store and internal/kb packages (#3)

move db types and CRUD to internal/store, export/import to
internal/kb. matches project structure used by serial-io-mcp,
terranoodle, and taipan-cli.

- internal/store: Document, DocumentSummary types, schema, CRUD
- internal/kb: ExportMarkdown, ImportJSON, Import
- main.go: thin handlers importing store and kb
- deleted flat db.go, export.go and their tests
