refactor: extract resolveEmptyDiff to fix gocyclo

handleFileLoaded cyclomatic complexity was 26 (threshold 20) after the
staged-only fallback addition. Extract the empty-diff resolution logic
(staged --cached retry + untracked disk read) into resolveEmptyDiff,
reducing complexity to 15.
