docs(skill): use ${CLAUDE_SKILL_DIR} instead of ${CLAUDE_PLUGIN_ROOT}

${CLAUDE_PLUGIN_ROOT} is only populated when a skill is loaded from the
plugin cache. When the same SKILL.md is loaded as a project skill,
personal skill, or from an additional directory, the variable is unset
and the launcher path collapses to "/.claude-plugin/..." → exit 127.

${CLAUDE_SKILL_DIR} is the documented portable substitution for this
case (see https://code.claude.com/docs/en/skills) — it always resolves
to the directory containing SKILL.md regardless of how the skill was
loaded, including for plugin skills.

Two occurrences updated: detect-ref.sh and launch-revdiff.sh.
