Address review: remove Semi break, fix test isolation, add regression tests

- Remove early `break` in HashJoin Semi path (both codegen and
  non-codegen); rely solely on `matched_` flag to deduplicate,
  fixing correctness when multiple build rows share the same key.
- TestExistsSubquery: save/restore neumann flag per iteration
  instead of mutating mid-test with hard-coded false at end.
- TestTpchWithData: explicitly enable neumann before q04 assertion
  instead of depending on global default.
- TestPushdown: remove dead `answer` plan strings; add the two
  nested scalar subquery SQLs to subqueryd_pushdown.sql regression
  tests (cross-level correlation and multi-table FromQuery cases).
