Enable -fvisibility=hidden (#56)

* [lib] Mark public APIs with default visibility

So that public APIs are kept visible when -fvisbility=hidden is enabled.

* [meson] Enable -fvisibility=hidden

Hide non-public symbols using -fvisibility so that you can use a
non-unity build, still run the tests, and keep the symbol table clean.
As part of this change, the tests need linking directly to the library
objects so they can access the new hidden symbols.
