Add ApplyServerExtensions hook to serve command (#4579)

* Add ApplyServerExtensions hook to serve command

Expose a package-level function variable in cmd/thv/app that enterprise
builds can set (via init()) to inject middleware and mount additional
routes into the ServerBuilder before the server is created.

This follows the same hook pattern used elsewhere for enterprise
extensions and requires no modification to this file by the enterprise
build.
