fix(vscode-forst): materialize @forst/cli with npm files layout for VSIX

Full-tree copy of `packages/cli` kept nested symlinks (e.g. dev `node_modules`) pointing at the monorepo, so `vsce package` still hit `invalid relative path: extension/../../package.json`. Rebuild `node_modules/@forst/cli` at both hoisted and local paths from `packages/cli` using the same `files` entries as `npm publish` (`dist/`, `README.md`, `LICENSE`, `package.json`), with `dereference` when copying `dist/`. Fail fast if `dist/index.js` is missing (`task build:cli`).
